When modules failed, I tried monolithic with similar results.
I just switched back to modules. Here are some appropriate messages:

gandalf:~ # dmesg | grep hd
Kernel command line: auto BOOT_IMAGE=lin2.4.10 ro root=302
BOOT_FILE=/boot/vmlinuz2.4.10 hdc=ide-scsi

scsi0 : SCSI host adapter emulation for IDE ATAPI devices

Looks to me like it is trying to use ide-scsi for hdc.... but
is not recognizing the presence of hdc.

got it.

boot    = /dev/hda
vga     = normal

  image  = /boot/vmlinuz2.4.10

I'll try to edit this a bit

alias block-major-8       sd_mod
alias block-major-11      sr_mod
alias scsi_hostadapter ide-scsi
options ide-cd ignore=hdc
alias scd0 srmod
alias scsi_hostadapter ide-scsi
options ide-cd ignore=hdc
